27.下列何種calcium channel blocker應避免使用於stable ischemic heart disease(SIHD)或acute coronary syndrome(ACS)?

A.amlodipine

B.felodipine

C.lercandipine

D.nicardipine

答案D

Ans:

依據uptodate 資料: Major side effects and safety of calcium channel blockers

Use of short-acting dihydropyridines has been demonstrated to cause reflex tachycardia and an increase in sympathetic tone, which may lead to poor outcomes in patients with heart failure and coronary disease. As such, these short-acting agents are generally no longer commercially available (or, if available, seldom used). However, the long-acting dihydropyridines (such as amlodipine, extended-release nifedipine, and felodipine) appear to be safe when used in patients with coronary artery disease or heart failure.

依據European Society of Cardiology資訊

DHPs such as amlodipine, felodipine and lercanidipine have greater vascular selectivity and minimal negative inotropic properties.
